• Headshot Questionnaire and Release Form must be completed Click HERE for PDF (I will also bring one to the shoot)
• Appointments must be made in advance

A $50.00 deposit must be paid to hold an appointment. (If the entire amount is not paid upfront)
(The deposit must be paid by PayPal or by mailing a Check or Money Order within 4 days from the time appointment is made)

• Remaining balance must be paid on the day of the shoot.
• Deposits are non-refundable.
